Licensed Social Worker – Full Time
Ancora Hospice is looking to grow the team! We are looking for a licensed social worker to support patients and families primarily in the Ada and Canyon County areas as our agency grows. Preferably with previous hospice experience, but not required.
Hospice social workers provide care in coordination with the interdisciplinary group (IDG) to assess for initial support needs of hospice patients and families with adjustment to living with a terminal diagnosis, grief and bereavement support, psychoeducation related to end-of-life care, advanced care planning, and referrals to community services to support care and comfort of patients and families.

Your Responsibilities
Complete bio-psycho-social assessment for new patients.
Develop care plan centered on patient and family identified needs.
Establish visit schedule as indicated by patients and needs.
Coordinate with interdisciplinary group to review care plans in accordance with CMS rules.
Conduct initial bereavement assessment to assess for identified bereaved.
Provide education on advance directives.
Assist in crisis management as needed with the IDG and patients.
Educate on housing or facility placement options and refer to community partners as needed.
Provide psychoeducation related to grief, loss, and end-of-life care.
Facilitate caregiver & grief support groups.
Provide supportive counseling related to grief and loss, psychoeducation for healthy coping, and bereavement.
Provide appropriate referrals to community resources.
Complete Military History Checklist to ensure veteran-centric care with assessment of needs.
Assist with other office or care-related tasks as assigned.
Complete documentation in accordance with agency protocol.
Adhere to state and local law, federal regulations, professional licensure regulations, and policy of Ancora Hospice & Palliative Services.
